What is Breast Revision?

Breast augmentation and breast reconstruction are among the most sought-after plastic surgery procedures, as they can revitalize the figure, expand clothing choices, and boost self-image. With millions of women undergoing breast procedures each year, it is not surprising that some will seek out revisions to address areas of dissatisfaction.

In some cases, the natural aging process may have compromised the implant appearance, necessitating a revision procedure. Other times, women are struggling with implant complications such as capsular contracture or rippling and need these concerns addressed. Finally, women may change their minds at some points about their implant size, type, or placement.

Breast revision can:

  • Enhance the results of the initial breast augmentation
  • Change the implant size, type, or position
  • Correct complications and asymmetry
  • Replace ruptured or leaking implants

Reasons for Breast Revision

Dissatisfaction with the initial augmentation surgery

The patient may be unhappy with the size, shape, or projection of her breasts and is seeking a final result that is more closely aligned with her original goals.

Rupture, leakage, or asymmetry

Patients may seek breast revision if a saline implant appears deflated or if an MRI has indicated leakage in a silicone device. Asymmetry is also a common reason for revisions.

Capsular contracture

This complication occurs when the scar tissue surrounding the implant hardens, compromising the implant’s appearance and causing discomfort for the patient. A capsulectomy, along with implant removal, may be needed.

Implant malposition

When the implants are too far apart, too close to each other, too high, or too low on the chest wall, a revision procedure may be an ideal solution.

Unhappy with implant size

Aging, pregnancy, and weight fluctuations can affect the breast’s volume. Over time, a woman may desire a change in implant size.

Implant rippling

As the breast tissue thins with age, rippling can occur. This concern can be addressed by repositioning the implant beneath the pec muscle and choosing a different implant type.

A change in aesthetic taste

Some women experience a change in their aesthetic sensibilities over time. They may wish to opt for a more natural look with a fat transfer breast augmentation instead of implants.

Implant removal

In certain cases, a woman may require the complete removal of her implants, as is common after BII (Breast Implant Illness) or BIA-ALCL (Breast Implant-Associated Anaplastic Large Cell Lymphoma).

Who is a Good Candidate for Breast Revision?

Any patient who is unhappy with the results from a previous breast augmentation or reconstruction may be an excellent candidate for our breast revision in Denver, depending on certain factors assessed during a private consultation with our team. Ideal candidates are physically and mentally healthy individuals who do not smoke and who are realistic in their expectations.

The Breast Revision Procedure

Breast revision is performed under general anesthesia on an outpatient basis and lasts 1-3 hours. For most patients, the surgeon can gain access to the chest through the same incisions used during breast augmentation. The old implant is removed along with any hardened scar tissue in the area.

If a woman desires larger implants, the surgeon will need to enlarge the breast capsule. If smaller implants are required, the capsule may need to be reduced using sutures. If the implant placement is changed, a new pocket may need to be created underneath the pec muscles, which will help reduce the risk of rippling and implant visibility.

Your surgeon may employ an acellular dermal matrix to maximize support to the chest scaffolding. If breast ptosis is a concern, a breast lift can be performed in tandem with revision surgery. Fat transfer can also be used to reduce implant visibility or to create a modest, natural-looking augmentation in patients who wish to have their implants completely removed.

Breast Revision Recovery

After surgery, you will awaken with your breast wrapped in dressings and small drains, which will help escort excess fluids out of the body. Once you are cleared, you will be escorted home with a responsible helper, friend, or family member who should assist you for the first 24-48 hours after surgery. You will leave with prescriptions for pain medications and a detailed, personalized aftercare plan.

Most patients will experience mild soreness, swelling, and discomfort for several days after their procedure. Ice packs can be placed on the breasts to help alleviate these concerns. You will return to the office within the week, at which time the drains and sutures will be removed. If dissolvable sutures were used, they will be absorbed on their own over time.

In the days after surgery, gentle walks are encouraged, as they can improve circulation, prevent clotting, and elevate your mood. Strenuous exercise and heavy lifting should be postponed for 2-3 weeks, or as recommended by your doctor. Swelling may persist for 1-3 months but will fade gradually. Scarring will fade over the course of 12 months.
